It is agreed that Paula Rebelo will provide child care_________ days per week at the rate of $__________ per week/day, payment due one week in advance.
It is also agreed that the parent will pay a late fee of $ per half hour or part there of for any child not picked up by the agreed upon time.

THE PARENT AGREES TO:
1) Pay the agreed rate for sick child days, statutory holidays including Easter Monday, child's holidays, five (5) days during the year when the provider will not be available, December 24 and the days between Christmas and New Years when the provider will not be available.
2) Notify the provider when the child/children will not be attending day care prior to the contracted drop off time.
3) Notify the provider if someone else will be picking up the child/children. Any person picking up the child for the first time will be asked for identification. Without prior notification the child will not be released to anyone other than the parent.
4) Supply diapers, formula, baby food etc. until such time as the child does not require them. Supply recess snacks for kindergarten children, and lunch and recess snacks for children remaining at school all day.
5) Provide an extra change of clothing, indoor shoes or slippers, towel, face cloth and toothbrush.
6) Dress the child with weather appropriate clothing to facilitate outdoor play at all times.
THE PROVIDER AGREES TO:
1) Provide quality care, promote and enhance all components of the child's total development by providing the child with an opportunity to develop in a safe, secure and nurturing environment.
2) Provide a child proof home that allows the child to freely explore his/her environment with as little restrictions as possible, conduct monthly fire drills, on-going safety checks on all equipment and toys used.
3) Not release the children in care to anyone other than the parent without their permission.
4) In the event of an accident, the Caregiver will attempt to inform the parent(s) as soon as possible.
5) The provider will not permit corporal punishment of any child; deliberate, harsh or degrading measures to be used on a child that would humiliate a child or undermine a child's self respect nor deprive a child of basic needs such as food, shelter or clothing.
6) Provide a "rest period" for each child five and under not exceeding two hours following the mid-day meal. A child who is unable to sleep during the rest period will not be kept in bed for longer than one hour and is permitted to engage in quiet activities.
7) Provide supervised daily outdoor play, weather permitting.
8) Engage the children in circle time, songs, games, crafts, field trips (occasionally an additional fee may be requested for this).
9) Provide nutritious morning/afternoon snacks and lunch where applicable.
THE PARENT(S) ACKNOWLEDGES(S) THAT THE HOME CARE CENTRE IS A SAFE ENVIRONMENT AND THAT IN THE EVENT OF AN ACCIDENT, THE CHILD CARE PROVIDER IS IN NO WAY LIABLE.
THIRTY (30) DAYS WRITTEN NOTICE MUST BE GIVEN BY EITHER THE PARENT OR CHILD CARE PROVIDER TO TERMINATE THIS CONTRACT. IF THE PARENT FAILS TO PROVIDE THIRTY (30) DAYS NOTICE OF TERMINATION, THE CHILD CARE PROVIDER SHALL RECEIVE ONE (1) MONTH'S FEE PAYMENT.
